Patients may request copies of their medical records by completing the AUTHORIZATION FOR RELEASE, USE AND DISCLOSURE OF HEALTH RECORDS (English) or AUTHORIZATION FOR RELEASE, USE AND DISCLOSURE OF HEALTH RECORDS (Spanish) and faxing it to the Health Information Management office of the location where care was received.
